The 53-year-old actor, best known for his role as Dr. Mark Sloan on Grey’s Anatomy, appeared in a guest role as Matthew Ramati — a firefighter struggling to tell his family about his diagnosis with amyotrophic lateral sclerosis, the progressive neurodegenerative disease that affects nerve cells in the brain and spinal cord.
Dane publicly revealed his own real-life ALS diagnosis earlier this year, making the storyline especially poignant. In April, he shared that he had been experiencing symptoms including weakness and difficulty with movement, and has since spoken openly about the emotional and physical toll of the disease.
In the episode, Matthew visits the medical team at Wolf Hospital after experiencing difficulty breathing and losing strength in his hands. Viewers watch as he battles the reality of a disease that steadily takes control away from those experiencing it. Even in distress, he delivers a moment of humor, joking:
“Fine, I’ll quit smoking cigars.”
Dr. Oliver Wolf, played by Zachary Quinto, encourages Matthew to face his fears and spend Thanksgiving with his loved ones. The episode closes with a heartwarming scene: a firetruck filled with fellow firefighters and those whose lives Matthew helped save arrives to support him in an emotional tribute.

He continues to work on HBO’s Euphoria, where he plays Cal Jacobs, and filming for season 3 wrapped earlier this year. However, he recently missed a planned Grey’s Anatomy cast reunion at the Emmys due to a fall caused by ALS-related balance issues.
The cameo has sparked calls for more appearances, with many viewers calling his performance “brave,” “raw,” and “one of the best in the series so far.”
