The original I Can Only Imagine told the story of Millard’s difficult childhood and his relationship with his abusive father, Arthur, portrayed by Dennis Quaid. The film centers on Bart’s son, Sam, who was diagnosed with Type 1 diabetes as a child. Millard recalls the early years as chaotic, with Sam’s diagnosis arriving the same week his daughter Gracie was born prematurely. In the movie, Bart struggles to balance his son’s fragile health with Sam’s desire to pursue music, creating tension between father and son.
Millard has spoken openly about the reality behind the story, noting that Sam endured more than 60,000 shots before advancements in medical technology eased his treatment. The film portrays Bart’s protective instincts and the strain chronic illness places on families.
Friendship and Faith
The movie also depicts Bart’s friendship with Christian singer Tim Timmons, played by Milo Ventimiglia. Timmons was diagnosed with incurable cancer in 2001 and given five years to live. His battle inspired the song Even If, co-written with Millard, which became MercyMe’s biggest hit after I Can Only Imagine.

Although Bart’s father died in 1991, his presence looms large in the sequel. Flashbacks remind Bart of Arthur’s transformation before his death and highlight Bart’s determination to be the kind of father his own dad never was.
